AI sustavi pariraju liječnicima u skrbi za pacijente, pretragama i liječenju

Razvijena je umjetna inteligencija (UI) sposobna ispitivati pacijente o njihovim simptomima, određivati potrebne pretrage, tumačiti rezultate, propisivati lijekove, pa čak i planirati hospitalizaciju. Također je stvorena još jedna UI koja prilagođava planove liječenja na temelju napredovanja bolesti i reakcija pacijenata nakon višestrukih posjeta, postižući rezultate usporedive ili više od liječnika u nekim kategorijama tijekom virtualnih kliničkih ispitivanja.

Rezultati istraživanja o MIRA-i, koju su razvili Tehničko sveučilište u Dresdenu i Sveučilišna bolnica u Heidelbergu u Njemačkoj, te AMIE-u, koji su razvili Google Research i Google DeepMind, zajednički su objavljeni u međunarodnom časopisu Nature 17. u mjesecu (po lokalnom vremenu). Dok su postojeće medicinske UI prvenstveno obavljale ograničene zadatke poput dijagnosticiranja bolesti ili analize medicinskih slika, ove dvije UI osmišljene su kao podrška cjelokupnom procesu upravljanja pacijentom — od uzimanja povijesti bolesti do odabira pretraga, odluka o liječenju i naknadne skrbi.

◇ MIRA: UI sposobna za pretrage, propisivanje lijekova i planiranje hospitalizacije

MIRA je UI agent koji djeluje u izoliranom virtualnom okruženju modeliranom prema bolničkim elektroničkim medicinskim zapisima. Komunicira s drugom UI koja preuzima ulogu pacijenta kako bi potvrdila simptome, povijest bolesti, trenutne lijekove, alergije i drugo. Zatim naručuje pretrage krvi, urina, mikrobiološke pretrage i snimanja te postavlja dijagnoze na temelju rezultata.

MIRA propisuje specifične nazive lijekova i doze, odabire operacije ili zahvate te odlučuje o hospitalizaciji pacijenata. Za razliku od konvencionalnih medicinskih UI koje daju nejasne odgovore poput “moguća upala pluća”, MIRA generira standardizirane naloge za pretrage i recepte kompatibilne s elektroničkim medicinskim zapisima. Da bi to postigla, oponaša rad bolničkih sustava primjenom međunarodnih medicinskih sustava kodiranja koji klasificiraju pretrage, lijekove i bolesti.

Prosječna dijagnostička točnost MIRA-e (87.8%) uspoređena je s četiri specijalista koji su ocjenjivali istih 311 slučajeva pod identičnim uvjetima, pri čemu je MIRA nadmašila rezultat specijalista od 78.1%. Sustav je slijedio klinički tijek rada sličan stvarnim hitnim prijemima, napredujući od razgovora s pacijentom do fizičkih pregleda, krvnih pretraga, snimanja i konačno odabira lijekova ili operacija.

◇ AMIE: UI koja prilagođava planove liječenja na temelju reakcija pacijenata

Googleov AMIE osmišljen je za praćenje promjena bolesti i reakcija na liječenje tijekom višestrukih ambulantnih posjeta. Čak i nakon postavljanja dijagnoze, liječnici moraju odlučiti o dodatnim pretragama, hoće li nastaviti s istim lijekovima ili ih promijeniti te kada zakazati kontrolne preglede. Ovaj dinamični proces donošenja odluka u središtu je AMIE-ovog fokusa.

AMIE radi s dvije specijalizirane UI:

  1. Jedna za interakciju s pacijentima.
  2. Druga za cjelokupno planiranje liječenja.

Izgrađen na Googleovoj generativnoj UI “Gemini”, sustav analizira prethodne medicinske zapise i trenutna stanja pacijenata. Pri donošenju planova pretraga i liječenja, AMIE se oslanja na smjernice referentnih institucija, poput britanskog Nacionalnog instituta za zdravlje i izvrsnost u skrbi (NICE).

Istraživači su stvorili 100 virtualnih slučajeva pacijenata u područjima kardiologije, pulmologije, ginekologije/opstetricije, gastroenterologije i neurologije/mišićno-koštanog sustava. AMIE-ova učinkovitost uspoređena je s 21 liječnikom primarne zdravstvene zaštite tijekom tri simulirana posjeta po pacijentu. Ulogu pacijenata preuzeli su obučeni glumci, a specijalisti su ocjenjivali transkripte razgovora i planove liječenja.

AMIE je također postigao bolje rezultate u specifičnosti, detaljno navodeći ne samo “propisati antibiotike”, već i konkretne nazive lijekova, doze, načine primjene, trajanje liječenja i rokove za kontrolne preglede. Ipak, dizajn AMIE-a koji generira dugačke, specifične odgovore možda mu je donio prednost u evaluacijama, pri čemu specifičnost ne jamči automatski stvarnu učinkovitost ili sigurnost.

◇ Uspjeh virtualnog ispitivanja… Potrebna daljnja validacija za stvarnu primjenu

Stručnjaci upozoravaju da se ovi rezultati ne smiju tumačiti kao “pobjeda UI nad liječnicima”.

Oba istraživačka tima naglasila su da UI sustavi još nisu spremni za samostalnu skrb o pacijentima. Prije primjene, potrebna su:

Dok je MIRA “akcijski orijentirana UI” koja provodi pretrage i liječenje unutar okvira elektroničkih medicinskih zapisa, AMIE je “UI za upravljanje bolestima” koja prilagođava planove kroz višestruke posjete. Ove studije naglašavaju kako se medicinske UI razvijaju daleko izvan osnovnih dijagnostičkih alata, krećući se prema sveobuhvatnoj podršci kliničkom radu liječnika.
